Services and Programs
Case Management: Case management services are
provided to assist the consumer in obtaining the basic
practical services required for adequate living. The
case manager and the consumer jointly identify social
service needs and assess how much or what sort of
assistance is needed. Jefferson Place staff is an
integral part of procuring the services required,
however, the consumer is encouraged to maintain these
services on his own behalf.
Emergency Shelter: Jefferson Place
provides, free of charge, a supervised and secure
emergency shelter for 150 homeless men. Each consumer is
allowed a 120-day stay, per year, with a 12-month period
between each stay. A day sleep period is provided for
those consumers that work at night and have submitted
required documentation as to their night schedule.
Follow-Up Services: Follow-along contacts
are maintained for all consumers who have successfully
completed the program at Jefferson Place. The Case
Manager seeks to have contact (face-to-face or
telephone) with the consumer at least twice per month
for the first three months and then once per quarter for
three quarters. The Case Manager documents each contact
and/or attempted contact in the consumer’s clinical
record. Follow-along contacts are supportive and attempt
to obtain information relative to job situation, living
conditions, life changes, usefulness of Jefferson Place
programming, and the current need for addition services.
Transitional Housing: This award-winning
program provides residential care to 50 non-drug using
homeless men. The maximum length of stay is 24 months.
The focus is geared towards coordinating a comprehensive
array of interventions and services in a single
treatment setting. The program provides support and
structure as the consumer rebuilds a substance-free
lifestyle, optimizes life functioning, addresses marital
and other family issues, enriches job-functioning and
financial management, and reintegrates them back into
the community. |
