Mission

To provide a continuum of care in collaboration with community partnerships of

homeless and at-risk women, youth, men and families resulting in their successful

 reintegration into the community.  We strive to enhance the continuum of care

services to the homeless and at-risk men, women, families and youth to regain a

state of independence that leads to a positive and productive reintegration into the community. Jefferson Place has established a continuum of homeless services including

emergency shelter, transitional housing, case management, secondary

substance addiction counseling, resettlement assistance, transportation, and

 follow- along services. Services are offered with the assistance of multiple

community partnerships. Jefferson Place operates 24 hours a day, Monday

through Sunday.

To enhance to the continuum of care services that will assist homeless and at risk

 men, woman, families, and youth to regain a state of independence that leads to

a positive and productive reintegration into the community.

Jefferson Place
